Accommodation:
- The duration of the stay according to the program can be arranged individually but cannot be billed by single days, only in packages (7, 14, or 28 days).
- Admissions take place at times arranged with the patient or their family; arrival is preferred between 8:00 AM and 12:00 PM. If arriving at other times, preliminary medical consultations on that day are not guaranteed.
- On the first day of the stay, qualification assessments are conducted, including an initial consultation with a physiotherapist and a general medical history.
Rehabilitation Treatments:
- On the first day of the stay, rehabilitation sessions do not take place or their duration is shorter.
- The total duration of rehabilitation with a physiotherapist and other therapies depends on the chosen program.
- A rehabilitation unit in the programs is 30 minutes.
- Rehabilitation and other therapies are selected individually for each patient; the chosen program only determines the number of units, and rehabilitation sessions are scheduled for each patient individually.
- All rehabilitation treatments and other therapies – the duration and number of treatments depend on the patient’s health condition, the absence of contraindications, and therapeutic recommendations.
- Occupational therapy and physical and psychological activation of the patient, including relaxation, increasing independence, improving manual skills, motor skills, memory exercises, ergotherapy, relieving nervous tension, and other therapeutic activities – group or individual work with the patient: according to the CONSTANCE CARE program from 8:00 AM to 4:00 PM, Monday to Friday.
- The overall rehabilitation time according to the chosen program may be subject to change due to the patient's health condition, any medical contraindications, or general health status. The medical staff will decide on any changes to the rehabilitation time.
Notes
The patient or their caregiver is required to submit a correctly completed qualification form (either in person or by email) no later than 24 hours before the patient’s arrival. If false or incomplete information regarding the patient's health condition is provided, CONSTANCE CARE reserves the right to refuse admission immediately. Admission decisions are made by the CONSTANCE CARE staff.
CONSTANCE CARE reserves the right to refuse admission if contraindications or other circumstances that limit the ability to safely and properly provide medical services are identified.
